Schedule a New Client Appointment
Schedule your first appointment. Please be aware that an intake will take about 1 hour and 30 minutes.
Fill Out New Client Paperwork
You will need to fill out several sections of paperwork. I know this can be a lot, but it's important to give as much information as possible! Go HERE for intake form.
Respond to New Client Email
Once you have scheduled and appointment and filled out the initial form, you will receive an email with instructions for your NewMind account and the remaining detailed intake forms. You can also respond to this email with any current labs or other test results. See the email for more details.
